When you are referred to Care Beyond Measure AU Pty Ltd (CBM), certain essential information about you is provided by the referring person or agency to enable us to provide the care requested. This information is collected and recorded either electronically or in hard copy in client files and includes your name, address, contact details and information about your reasons for requiring care from our organization. It may include information about your health.
Every time you contact CBM, new information may be added to your record. This allows all staff involved with the coordination and management of your care program to access information easily and quickly when it is needed. We need to collect and keep this information to provide you with a quality service, which meets your assessed care needs.
All information (including Health Records) is stored securely at CBM. Electronic information is password access controlled and doubly secured from unauthorized access. CBM is committed to ensuring that records are maintained, accurate and up to date. Hard copy data is stored in secure, locked filing cabinets. You can request to see any information held on your file. This may include viewing the information on premises with a staff member present, getting a summary or in some cases, a full copy of your records or having information explained to you if you wish. There may be particular circumstances that prevent us from releasing information. If this applies to your records, the reasons will be explained to you.
A fee may be charged (in line with Government guidelines) for the provision of the information.
You have the right to amend any information in your records that you consider is incorrect, incomplete or misleading. You cannot ask that information held on your file or health record be removed or destroyed although you may ask that a correcting statement be added to it.

If a person is unable to give consent about the release of their information (due to age, physical or cognitive limitations), a decision will be sought from that person's authorised representative.
In order to provide you with more comprehensive and effective services, it may be helpful to share your information with other service providers (e.g. doctor, health centre). In this situation, you will be asked to give consent for us to disclose any necessary information from your health record to the other party to help them assess and meet your needs. Please note that you do have the right to refuse consent if you wish. However there are some circumstances where information may be shared without your consent.
When emergency treatment is required when legal requirements deem it necessary (e.g. subpoena of client records; for law enforcement purposes).
In these circumstances, you will be advised as close as we can to the time when information is released.
